Description:

Volunteer Position Description

As a Healthcare Navigator you are often the first person a patient sees at the clinic. You will help with the following tasks:

  • Help enroll patients into Patient Portal
  • Inform patients about the Patient Portal and its features
  • Inform patients about the Health Education resources available at VFC
  • Research other resources that would be helpful VFC to patients and coordinate with VFC staff to make such resources available in the Waiting Room
  • Assist by conducting patient surveys on behalf of VFC and affiliated programs
  • Track data, including
    • Number of patients enrolled per shift
    • Number of patients that did not have email accounts / Number of accounts created

Volunteer Commitment

Volunteers must be willing to commit to one shift per week for 6 consecutive months.

Requirements

  • Must attend an Information Session
  • Complete all required documents
  • Complete Registration Day/Interview
  • Complete Healthcare Navigator Training
